I submit this application for LU Cheer/Dance Team tryouts.  I give the right to the advisor and/or the E-Board to check my grade point average to make sure that it is at least a 2.25.  In submitting this application I understand that if I am selected to the LU Cheerleaders/Dance Team I accept the following:

My GPA must remain at least a 2.25 every semester, or I will be placed on probation from the team until my grades come up.

I will attend the scheduled practices, performances (games, pep rallies, parades, competitions, etc.), community service events, & other team appearances.

I will pay all financial requirements.

I understand & accept that failure to meet any of the items above will result in my disqualification or removal from the team.  I also agree that if selected I will fully abide by the LU Cheerleaders/Dance Team Constitution.
