Mid Course Reports, Facilitator Invoice, and Group Life Issues

  • The mid course report form and all facilitator invoices are found on the LIM documents web page www.lim.loyno.edu/extension/documentsin the facilitator [liaison] section under common curriculum session five forms. The focus course invoices are found under focus course documents and the discernment process invoice is found under discernment process.
  • All these invoices and forms may be emailed as WORD attachments to Ellen Casanovas at ecasanov@loyno.edu or faxed to 504-865-2066. In the Re: section of the email/fax please put, title of the invoice, your last name, sponsoring agency, and course title. Please keep a copy of the completed forms for your records. ALL the requested information must be completed on the invoice. Incomplete invoices will not be able to be processed, and will be returned to facilitators.
