Procedure: Form Creation and Programming

I need an online form, application and/or survey. How do I get one? What can my ETA reasonably be?

Quick and easy forms

I have some web experience and am somewhat familiar with making online forms. How can I make a simple form quick, using Loyola's branding?

If you need a simple form, application, or survey quick and have some knowledge on making web forms, the program phpForms will serve you well. phpForms allows you to create a simple form with text fields, text areas, drop down menus, check boxes and many other field types. You do not need to know how to program with HTML code. For each field needed, you simply provide the field name, field type, default values, and if the field is required. phpForms allows you the ability to have successful form submissions to e-mailed to one address or multiple e-mail addresses. You may also have the data stored in a database, so that results can be exported to an Excel spreadsheet.

If phpForms seems to be a solution, please fill out a Web Request Form to be trained on the program. Be sure to tell us the date and time when you are available to be trained.

Training can usually take place by 1-2 weeks. Project completion will then be dependent on you.

phpForm help

I have a phpForm already, but need help updating or troubleshooting. Who do I contact?

Please fill out the Web Request Form. Most updates and troubleshooting can be accomplished within 1 week.

Complex forms

I need a complicated form and/or have no knowledge about web forms.
Or…
I have a MS Word doc or PDF file form / application, and want to make it an online form / application.

The Office of Web Communications can create customized web forms. In filling out the Web Request Form, certain criteria must be met. Examples of the form can be attached in the web request form as a MS Words doc or PDF file.

Please specify:

  • Introductory text, if needed
  • Fields needed
    • This includes submitter's contact info, form questions, etc…
  • Fields required to successfully submit the form, application, or survey.
  • Do you or others need to be notified of successful submissions with a copy of the filled out form?
    • If so, specify e-mail address(es)
  • Does the submitter need to be e-mailed a confirmation e-mail?
  • Thank you text after form is filled out, if needed.

Customized forms with email submission can be created in 2 weeks. When filling out the Web Request Form, please allow for this time constraint when selecting the completion date needed.

Database interaction

I need more than just an email of successful form submissions. What are my options?

The Office of Web Communications can program successful form submissions to be stored in a database. Database interaction does require more resources and time. In asking for form submissions to be stored in by a database, please keep this in mind.

Retrieving form data from a database

I want or have my forms interacting with a database. How do I retrieve / view my data from the database?

If your form interacts with a database, there are a couple of options that you can choose from in retrieving the data.

  • Export data to an Excel spreadsheet.
    • This is the less time consuming option for database interaction. When data needs to be viewed in an Excel spreadsheet format, contact the Office of Web Communications via the web request form. This request can normally be met within 1 week.
  • Create a custom Admin Dashboard
    • An admin dashboard is the more advanced approach and is usually more suited for the needs of important applications or custom marketing campaigns.
    • The Admin(s), you or who ever specified, will be able to view the data online. Using the Admin Dashboard you can query and search results, quickly gather statistics, and perform other custom options as needed.
    • A customized Admin Dashboard does add significantly to programming and training time. Please allow 8 weeks for completion.

If either option is needed, please specify in the Web Request Form, understanding the extra time needed to complete the project. The Office of Web Communications programmers will contact you to discuss the project in more detail.
