I need an online form, application and/or survey. How do I get one? What can my ETA reasonably be?
If you need a simple form, application, or survey quick and have some knowledge on making web forms, the program phpForms will serve you well. phpForms allows you to create a simple form with text fields, text areas, drop down menus, check boxes and many other field types. You do not need to know how to program with HTML code. For each field needed, you simply provide the field name, field type, default values, and if the field is required. phpForms allows you the ability to have successful form submissions to e-mailed to one address or multiple e-mail addresses. You may also have the data stored in a database, so that results can be exported to an Excel spreadsheet.
If phpForms seems to be a solution, please fill out a Web Request Form to be trained on the program. Be sure to tell us the date and time when you are available to be trained.
Training can usually take place by 1-2 weeks. Project completion will then be dependent on you.
Please fill out the Web Request Form. Most updates and troubleshooting can be accomplished within 1 week.
The Office of Web Communications can create customized web forms. In filling out the Web Request Form, certain criteria must be met. Examples of the form can be attached in the web request form as a MS Words doc or PDF file.

Customized forms with email submission can be created in 2 weeks. When filling out the Web Request Form, please allow for this time constraint when selecting the completion date needed.
The Office of Web Communications can program successful form submissions to be stored in a database. Database interaction does require more resources and time. In asking for form submissions to be stored in by a database, please keep this in mind.
If your form interacts with a database, there are a couple of options that you can choose from in retrieving the data.
If either option is needed, please specify in the Web Request Form, understanding the extra time needed to complete the project. The Office of Web Communications programmers will contact you to discuss the project in more detail.
