IMMUNIZATION REQUIREMENTS FOR FALL 2009 REGISTRATION
Students who do not meet registration requirements will not be allowed to attend classes in the fall. In order to determine the status of your immunization record, sign in to your LORA account, click Login to Student Services (LORA), go to Personal Info and click on the link View Immunization Status, Ethnicity, and Race. A screen will appear and will provide the status of your records.
The following immunizations are required:
- Tetanus or TDaP (within the last 10 years)
- Two doses MMR (not needed if born before 1957)
- Meningitis (must have received vaccination in 2005 to present; good for 10 years)
Instructions for Meeting Immunization Requirements for Fall 2009 Registration
- From the Loyola Student Health Services website, click on Immunization Forms.
- Print the Immunization Complance Form, which must be completed and signed by a physician. Immunization records without a physician's signature WILL NOT be accepted.
- A waiver form must be completed for any vaccination you do not wish to receive. Waiver forms for all vaccines are available on our website and must be sent with your Immunization Compliance Forms in order to meet registration requirements.

Instructions for Releasing Immunization Records from Loyola Student Health Services
- From the Immunization Forms page print the Record Release Form.
- Complete, sign and date the Record Release Form and mail or fax the form to the address above.
