The Information Release and Promissory Note are required documents each semester. Please click on the instructions below before completing the forms.
Please note that the promissory note and 10% deposit are due no later than thirty days after the semester has started. The promissory note linked below is for the most recently billed semester.

(The promissory note must be executed in the presence of a Loyola witness or a notary public. Faxes, copies and emails are not acceptable legal promissory notes. If notary public is used, please mail the original signed documents to your Dean's office.)
