NEC DTERM Series I -- TELEPHONE LABELING INSERT - PRINT REQUEST

The NEC DTERM Series I telephone faceplate can be custom printed by the Telecommunications Department to suit your needs and give your phone a clean professional look. Just fill out the form below and let us do the rest. Please allow up to 30 days for delivery of your new label insert to be delivered to you via campus mail.

The four rows of boxes below, correspond to the four rows of buttons on your NEC DTERM phone. The top two rows are for telephone number line appearances only. Please enter them EXACTLY as they appear on your set. The bottom two rows are for speed call use only.

On the first two rows, enter the 4 digit extention to which each is assigned.

On the second two rows, enter up to 6 characters each for your speed dial assignments.

Be sure to enter your telephone extension and your name in the appropriate boxes.

Click on "SUBMIT YOUR REQUEST" , and you are done! We will mail you your new label.
