Authorization for Use of

Federal PLUS Loan Proceeds

Pursuant to federal regulation, unless other authorized, PLUS loan proceeds (original loan amount minus insurance fees) may only be applied to “allowable instiutional fees.” At Loyola University, these allowable fees are limited to tuition, fees, and campus housing.

In order to have these proceeds applied to “other” charges (e.g. meal plan, books, etc.), we must obtain the Parent Loan borrower’s authorization.  Please complete the following form.

I authorize Loyola University New Orleans to apply PLUS loan proceeds, in excess of allowable institutional fees, to “other” charges.  If excess loan proceeds exist after application of funds as indicated, I authorize Loyola University New Orleans to refund this excess directly to the student.  Student's Name:
