1. Please complete by May 5, 2007 in order to maintain recognition as a student organization of Loyola University. After May 5 there is a $15 late fee to recharter your organization.

2. Only "Website Address" and "Membership Demographics" sections are OPTIONAL. All other fields must contain correct data in order to submit this form.

3. Contact information is not shared outside of Loyola University. The organization and president's email account will be published in the Student Organization Directory and Loyola Webpage. President's phone number will not be listed.
