Acknowledgement of Risk and Waiver of Liability
Introduction:
The LUCAP Acknowledgement of Risk and Waiver of Liability Form, or the LUCAP Liability Form, is a fact of life for current volunteers. Our work environment simply isn't the same as it was before Hurricanes Katrina and Rita, and Risk Management stipulates that volunteers must be informed of the risks associated with a LUCAP project and agree to waive Loyola University New Orleans of any liability in the event that something unfortunate happens.
Everyone has to fill out this form, even the project leaders. In fact, everyone needs to fill out a form for each project, whenever that project goes out. Always.
Yes, it's annoying. Yes, it feels like a waste of paper. However, there's a bright side!
The Benefits:
These forms, though we didn’t ask for them, provide us with unprecedented record-keeping capabilities. They'll streamline headcounts when traveling, allow us to call you if you get lost, and eliminate the guesswork regarding questions of who did what for how long on which dates. Beyond those individual benefits are the organizational benefits for LUCAP. For example, with the forms we can extract quantifiable data that allows us to say "On September 9, 2006, 45 volunteers completed a total of 247.5 hours of community service in the form of house gutting and floating sheetrock. The hourly wage (mean) for this type of work in Louisiana is $15.70, resulting in a volunteer contribution of $3885.75 for that event." The credibility gained from these types of statements is usually reserved for professional service organizations.
The Shortcut:
It's a hassle to fill out the whole form over and over right before your project leaves, but if you download the liability form to a computer with a printer, you can save a lot of time by editing in your information.

- Be sure to leave the"Description of Activity" field and the "Signature/Date" fields blank. Those MUST be handwritten.
- Once everything else is filled out, print it out, but please save paper by printing the two pages on one side of a page. To do this in Microsoft Word, select "Print" from the File menu, when the Print window appears, adjust "Pages per sheet" to 2 pages, and then click "Print."

- If the formatting is alright, print out several more copies for future use.
